Please fill out this form and complete the booking steps to make an appointment.

Booking Procedure:

  • Step 1: Fill out the appointment request form

  • Step 2: Complete the payment

  • Step 3: Wait for confirmation email, receive in 48 hours

  • Step 4: Fill out the application form and attend the appointment

Notes:

  • Appointments will only be arranged once the payment is received for all initial sessions.

  • For urgent sessions within 3 days, please contact us for availability.
